I hereby authorize Cocoa Expo Sports Center to charge my credit card that I have listed above. I understand that once my team is booked there will be no refunds issued. I understand that in order to fill out this application, I must be 18 years of age or older. By entering your name below, you are certifying that you meet the age restriction. REFUND POLICY: Once your team has been accepted into the tournament, there will be no refunds given. If your team chooses not to participate after you have been accepted, you will be given the option to transfer the balance towards the entry fee of a future tournament. WEATHER CANCELLATIONS: The game guarantee does not apply to games not played due to forfeits or inclement weather. Under normal circumstances each team will play the guaranteed number of games; however Cocoa Expo is not responsible for teams that forfeit games or games lost due to inclement weather. Teams will receive credit for a future tournament if less than (1) game is played. There will be no credit issued once the second game begins. UNABLE TO FILL AGE DIVISION: If not enough teams are registered within a specific age bracket, the Tournament Director will notify the participants as soon as possible, and the participants will be given the option of playing up in age division or receiving a full refund of their entry fee.
